Medical Advice Disclaimer
The information provided by Elevate Wellness, including services from our Nurse Practitioners, is intended to supplement—not replace—your existing healthcare. Our practitioners offer medical advice based on the information you provide, but if we are not your primary care provider (PCP), we may not have a complete picture of your overall health history. Always consult your PCP for a comprehensive understanding of your medical needs.

Consultation Fees
Initial consultation fees are non-refundable. If you proceed with our weight loss services, the consultation fee will be deducted from your initial appointment.

Appointments & Follow-Ups
Follow-up weight loss appointments are required every 4 weeks for continued medication use, with Telehealth options available for every other session. We aim to make your access to care as easy and convenient as possible.

Monthly Medications
Our GLP-1 medications are prescribed through a partner pharmacy starting at $150 per month. Please note that we cannot guarantee insurance or HSA coverage for these prescriptions. Consult with your provider for specifics on coverage.

Termination of Services
Elevate Wellness reserves the right to terminate services if terms are not met or if the treatment plan is not adhered to.

Health Savings Account (HSA) providers for our services. Coverage and approval depend on individual insurance
